Example: Submitting claims to Medicaid, Medicare, or private insurance providers.
Healthcare Operations
We may use PHI for administrative and operational purposes, including:

• Licensing and accreditation
3. OTHER PERMITTED OR REQUIRED DISCLOSURES

Request a list of certain disclosures made outside treatment, payment, or operations.

You may request a paper copy at any time.
6. TELEHEALTH & ELECTRONIC COMMUNICATIONS
New Leaf Wellness Center may provide services via telehealth platforms and electronic communications that meet HIPAA security standards. While we take reasonable safeguards to protect your information, While we implement industry-standard safeguards, no electronic transmission is completely secure. By using electronic communication, you acknowledge and accept this risk.
7. OUR RESPONSIBILITIES
NKE New Leaf LLC will:
• Maintain administrative, physical, and technical safeguards
• Limit access to PHI to authorized workforce members
• Maintain Business Associate Agreements with service providers who access PHI
• Comply with federal HIPAA regulations and applicable Texas privacy laws
